該神經發源自延髓的舌下神經核，並從延髓的橄欖和錐體之間的橄欖旁溝穿出，然後經行舌下神經道（Hypoglossalcanal）。

從舌下神經道穿出後，舌下神經與頸椎C1的前側支匯合，然後纏繞迷走神經，並從內側頸動脈和內側頸靜脈之間通過。

在通過二腹肌後腹的深部後，舌下神經進入舌。

通常可通過讓病人伸舌來檢查舌下神經的功能。

如果一側的舌下神經功能有障礙，舌伸出時將偏向有障礙的一側。

舌下神經的強健度可通過使病人將舌頭頂向面頰，並由醫生用手指觸動來檢查。
